Management of Intermittent Exotropia of the Divergence Excess Type: a Teaching Case Report
نویسنده
چکیده
Intermittent exotropia (IXT) is the most common form of childhood exotropia 2 with an incidence of 32.1 per 100,000 in children under 19 years of age. The strabismus is characterized by an exodeviation of one eye that is interspersed with periods of ocular alignment. Reliable measurement of the deviation is often hindered by the variable nature of the strabismus, and without careful observation and evaluation IXT can often be missed. Divergence excess (DE) is a type of IXT that is characterized by a larger magnitude exodeviation (phoria or intermittent or constant exotropia) at distance than at near. This report is of a 5-year-old patient with IXT of the divergence excess type managed by overminus therapy. Discussion includes classification of IXT, important clinical findings, diagnostic considerations and management with an emphasis on overminus therapy. The case report is intended for thirdand fourth-year optometry students.

متن کاملSelective surgery for intermittent exotropia based on distance/near differences.
BACKGROUND Classic teaching suggests that surgery for intermittent exotropia should be based on distance/near differences. Divergence excess, according to tradition, should be treated with symmetric lateral rectus recessions; simulated divergence excess and basic deviations should be treated with a recess/resect procedure. This teaching, to our knowledge, has not been systematically tested. O...

PURPOSE Classic teaching suggests that surgery for intermittent exotropia should be based on distance/near differences. True divergence excess exotropia should be treated with symmetric lateral rectus recession. The aim of this study was to investigate the effect of large bilateral lateral rectus (LR) recession in large-angle intermittent exotropia. METHODS Thirty-three consecutive patients w...
متن کاملA Tale of Intermittent Exotropia and Amblyopia – Does Vision Therapy Help?
Among the types of exotropia in childhood, intermittent exotropia (IXT) has been reported to be the most common type. In a population-based cohort, the prevalence of exotropia has been reported to be 1%, with convergence insufficiency and intermittent exotropia being the commonest of the subtypes. A recent large population-based cohort of 5831 preschool children reports a prevalence of 3.2% of ...
